Kennedy Type 5.

Comments:
The discus of this type of lamp was often deliberately broken in antiquity. Although it has been speculated that this was done in order to erase any discus motifs that might offend the religious sensibility of the owner, the likely reason for the practice in many cases is simply that in a region accustomed to the convenience of large filling apertures, a discus with a small one was regarded as an unnecessary hindrance.

See the comment under a factory lamp (which usually have no iconography at all).
